What are the requirements for a 'gold star'?

I completed the first scenario (Wadi Akarit), completing all of the objectives including the bonus ones,
but I only got a silver star for it. I want to try for the 'Gold Star' achievement, and I had thought that all that was required for a 'gold star' in a scenario was to complete all of the objectives. What is it I'm missing?

Primary objectives have to be taken by a specific turn number to get the gold star.
Bonus objectives are irrelevant. They are only for bonus points.
